R.E.A.L. Outreach Program

(Resilience. Empowerment. Action. Leadership.)

Powered by St Claire Family Homes

R.E.A.L Outreach is not a service — it’s a presence.

  • Where We Show Up

We show up where youth are: schools, neighborhoods, bus stops, gyms, street corners — any space where vulnerability meets potential.

Our goal is to reach the most overlooked youth: foster kids, probation youth, homeless teens, and those caught between survival and self-destruction.

We meet them with respect and consistency.

  • What We Do:

Street-Level Outreach to engage youth in crisis or disconnection

School Drop-In & Prevention Visits to identify at-risk students early

Mobile Resource Support including food, hygiene, and transportation assistance

Mentorship & Life Navigation for youth dealing with trauma, probation, or system failure

Crisis Intervention & Safety Checks in partnership with local orgs

What Makes R.E.A.L. Different?

We don’t just care — we’re clinically backed to act.
Every outreach team member at R.E.A.L. operates under Agency Affiliated Counselor (AAC) status, working directly under licensed clinical supervision to ensure ethical, trauma-informed, and effective support.

Our teams are certified in:

✔️ Mandated Reporting
✔️ HIPAA & Confidentiality
✔️ Youth Mental Health First Aid
✔️ CPR & First Aid
✔️ Trauma-Informed Care (ACES)
✔️ De-escalation & Crisis Intervention
✔️ Cultural Competency & Bias Awareness

We combine lived experience, professional training, and clinical oversight — so when a young person meets us, they’re not just being seen. They’re being supported the right way.
